Dental care without dental insurance
The SDG Dental Membership Plan is an annual membership offered directly by Silverlake Dental Group for patients without active dental insurance. It is not insurance. Benefits begin immediately, with no waiting periods, deductibles, claims, or annual maximums.

Available to patients with no active dental insurance only. All enrolled family members receive the same benefits.
Included care for each member
- Two preventive visits per year: each includes a comprehensive exam with Dr. Barkhordar, a professional prophylaxis cleaning, and checkup X-rays.
- One emergency exam per year: your exam is included when an unexpected dental concern arises.
- 3D diagnostic scan and consultation: included when exploring clear aligners such as Invisalign or cosmetic veneers.
10% off additional treatment
Members receive 10% off fillings, crowns, bridges, onlays, dental implants, veneers, teeth whitening, cosmetic treatments, clear aligners and Invisalign, and any additional cleaning or exam beyond the two included visits. Discounts apply immediately.
Additional treatment receives the member discount rather than being fully covered. Some restrictions may apply; ask the office about your specific treatment before enrolling.
Common questions
Is this dental insurance?
No. This is a direct membership with our office. You pay the practice directly, without an insurance claims process.
Who qualifies?
Patients without active dental insurance can enroll. If you lose your insurance coverage, you are welcome to ask about joining.
When do my benefits start?
Immediately. You can enroll and schedule your first included visit on the same day, subject to appointment availability.
Can I add family members?
Yes. Plans cover two, three, or four members, with additional members at $250 per person.
